As part of the assessment, I gather information from multiple sources, including teachers and caregivers, to gain a complete understanding of the patient's functioning across different settings. To provide the most accurate and comprehensive evaluation, it is helpful for Dr. Erickson to have access to previous medical records that are relevant to the primary concerns, history of present illness (HPI), medications, and specific referral questions. You can have the patient complete a Release of Information (ROI) form through your office and fax the completed ROI and key medical notes to us at 616-333-4927.

Detailed Reports and Recommendations

After the evaluation, we provide a detailed report that includes a summary of findings, diagnostic impressions, and personalized recommendations. These recommendations may include specific interventions, accommodations, and strategies to support the patient's development and well-being. We also offer follow-up consultations to discuss the findings and answer any questions you may have.
